Health Department Vital Records

Vital Records Fees (Cash, Local Check, Money Order/Cashier’s check for out of county residents, Debit/Credit cards*)

• Birth Certificates: $15.00 each
• Amendment Affidavits (includes one birth certificate): $20.00 each
• Death Certificates: (includes Coroner’s Education Fund): $18 each
• *A convenience fee of 3% is added to each debit/credit card transaction. A minimum convenience fee of $1 is added to transactions less than $33.
