Our Community Paramedic provides in-home support and connections to services to help residents stay safe and healthy. Contact the Community Paramedic for assistance with:
- Medication concerns (questions about safety, compliance, or difficulty obtaining prescriptions)
- Repeat falls or fall-risk concerns
- Connecting to home resources such as home health, physical therapy, and or transportation
- Complex medical conditions (ventilators, tracheostomies, LVAD, etc.)
- Infant wellbeing, including car seat installation and safe sleep education
- Chronic disease management support (CHF, COPD, diabetes, stroke recovery)
- Connecting individuals to mental and behavioral health resources
- Prenatal and postpartum support needs
- Assistance accessing vaccinations (home bound or no primary care provider)
- Health Education, including CPR & Stop the Bleed
PEACH Program (offers in-home support for moms and babies, focusing on maternal and infant health, education, and connection to resources)
